Violations: 6-1A, Violation of Code: [.04(6)(f) ] White and yellow American cheese, cottage cheese, and cheddar cheese holding in the upper portion of the prep cooler between 47°F-49°F since 7:30 AM, temped 9:30 AM. Cold-held potentially hazardous foods must be maintained below 41°F. The employee transferred foods to the reach-in cooler.  Corrected On-Site.  Repeat Violation.

Violations: 3-1A, Violation of Code: [.04(2)(a) ] Habiba's bean pies stored in the front display cooler for consumers are from a distributor who does not have a permit from the Department of Agriculture or local Health Department. According to the distributor, the pies are made in her home. All foods given to the general public must be from an approved source. The employee decided to discard the foods.  Corrected On-Site.  Repeat Violation. 4-1A, Violation of Code: [.04(4)(c)1(i)-(iii),(v),(viii) ] An employee was observed handling raw chicken tender with gloved hand then handled cooked sub roll with the same glove. Raw animal foods must separated from each other during storage, preparation, holding, and display. The employee decided to discard the food.  Corrected On-Site.  Repeat Violation. 6-2, Violation of Code: [.04(6)(g) ] Precooked ham and turkey removed from the freezer Saturday without thaw dates. Refrigerated, ready-to-eat, potentially hazardous food prepared and held in the establishment for more than 24 hours must be clearly marked to indicate the date by which the food must be consumed, sold, or discarded. The employee decided to place the dates on the foods.  Corrected On-Site.  New Violation. 6-2, Violation of Code: [.04(6)(h) ] Two packages of cooked original chicken with preparation date of 12/9/13 in the walk-in cooler. Correctly date-marked refrigerated, ready-to-eat, potentially hazardous foods must be discarded within 7 days from the date of preparation. The employee decided to discard the foods.  Corrected On-Site.  Repeat Violation. 16A, Violation of Code: [.06(1)(h),(i) ] 1. Hot water is intermittently at the hand sink in the prep area. 2. Hot water on the left side of the 3-compartment side is not working. Hot and cold water must be provided to all sinks.  New Violation.

Review with all staff.  New Violation. 2-2B, Violation of Code: [.03(5)(j)1&2 ] Multiple employee drinks found in unapproved containers and stored in unapproved location. Had person in charge collect all drinks. Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed.  Corrected On-Site.  New Violation. 5-2, Violation of Code: [.04(7)(e) ] Raw shelled eggs offered undercooked not marked with asterisk. Foods offered raw or undercooked must be asterisked as per consumer advisory requirements. **DISCLOSURE statement, REMINDER statement must be on the same page as the menu item that is offered raw or undercooked. Both statements must be in CAPS. Handout provided. Address.  New Violation. 6-1A, Violation of Code: [.04(6)(f) ] Cold-held potentially hazardous foods not maintained below 41oF; no time controls/documentation in place. Mashed potatoes, eggs DISCARDED. Raw beef, shrimp, fish, chicken salad moved to walk in cooler. Must hold at 41 deg. F or less.  Corrected On-Site.  New Violation. 9-2, Violation of Code: [.04(6)(j) ] Facility canning (water bath) pickles, okra, bacon jam (contains bacon). This is considered reduced oxygen packaging & requires a variance from the State of Georgia and a HACCP plan. **Owner did not discard product. Owner wanted to take product home for personal use.  New Violation. 15A, Violation of Code: [.05(6)(q) ] Ambient air of drawer cooler at 49 deg. F. Must have an ambient air of 41 deg. F or less AND must hold food at 41 deg. F or less.  New Violation. 15C, Violation of Code: [.05(7)(d) ] Fan in kitchen with dust build up. Must clean fan.  New Violation.
