Violations: 2-2A, Violation of Code: [.03(4)(a)-(f) ] The food employees were unaware of the reportable symptoms and illnesses. Each food employee must be aware of the reportable illnesses and symptoms. Each food employee was informed.  Corrected On-Site.  New Violation. 4-2A, Violation of Code: [.04(4)(c)1(iv) ] 1. Uncovered plantain chips on the storage shelf and stove; 2. Uncovered empanadas in the freezer; 3. Uncovered seasonings stored on the prep table; 4. Uncovered alcoholic pour spouts at the bar. Foods must stored in packages, covered containers, or wrappings when not in use. The items were covered.  Corrected On-Site.  New Violation. 4-2B, Violation of Code: [.05(7)(a)1 ] 1. Frying pans stored on the shelf of prep table across from the steam well had visible cooking oil present; 2. the knives were stored on the magnet with visible food debris present; 3. glass dipping jars with cooking oil residue present stored next to single-use items on the shelf at the entrance of the preparation area; 4. the baking dishes stored on the steam well with spider web present; 5. interior lid of the ice bin in the bar area had mold like substance present. All food-contact surfaces must be clean to sight and touch. The items were removed for cleaning.  Corrected On-Site.  New Violation. 10D, Violation of Code: [.04(8)(b) ] Packages of hamburger buns in the walk-in freezer with use-by dates of August 1, 2013; and loaves of breads with use-by dates of Wednesday stored in the freezer. Foods must be used by the use-by dates. The foods were discarded.  Corrected On-Site.  New Violation. 14A, Violation of Code: [.04(4)(k) ] 1. Bowls used for measuring rice were stored in water of 115 degrees Fahrenheit on the steam well; 2. The multi-use utensil for potentially hazardous foods were 92 degrees Fahrenheit on the stove. In-use utensils for hot-held potentially hazardous foods must be stored in a container of 135F water. The items were removed for cleaning.  Corrected On-Site.  Repeat Violation. 18, Violation of Code: [.07(2)(m) ] Rear screen door to restaurant has a gap along top part of the door and does not close tightly. All outer openings must be self-closing, tight-fitting doors.  New Violation. 18, Violation of Code: [.07(5)(k) ] Flies were noted in the kitchen. All outer openings must be protected to minimize pest in the establishment.  New Violation.
