Violations cited: A bowl for portioning raw beef was inside a container of fresh mushrooms next to the raw beef in the prep cooler. The bowl had raw meat and blood in it. Some foods were uncovered in a cooler. A container for holding cooked chicken was dirty. Garlic in oil wasn't kept cold. An employee was seen cooking rice without a hair restraint.
